This is for the RipTide modem/Sound Card in the Pavalion 4443 computer sold through Walmart. It works for Win95 through Win2K. Not sure about WinNT, though. It was a bear to find, and it's not on the recovery disk, unless you blow away the entire hard drive and return to HP standard.
